We are committed to protecting your privacy.
If you supply any personal information to us in the course of accessing health care services, or using our
online booking services, you consent to the collection, use and disclosure of your personal information
by us for the purposes stated at the time of collection in accordance with: (i) this Policy, and (ii) the
Health information Act and other applicable laws and regulations.
For appointment booking services, we collect your name, phone number, email address, the type of
exam you wish to book, as well as the requisition that is required for us to complete our diagnostic
imaging or other health care services. In the course of providing health services to you, we collect health information, including diagnostic, treatment and care information about you, as permitted under the Health Information Act.
We use your health information for the purpose of providing health services to you, including diagnosing
and treating illness and medical conditions. We will not collect, use, or disclose your health information
except for purposes authorized by the Health Information Act, our policies, and other applicable law.
We use health information to provide you with health services, communicating with other health care
professionals, as permitted under the Health Information Act, and billing Alberta Health or other health-
care providers for health services. For example, we disclose our report to your referring physician in
order for them to provide continuing care to you.
Our administrative staff will use booking information solely for the purposes of booking your
appointment. The information you provide is not disclosed to any third party unless that disclosure is permitted under the Health Information Act, or other applicable law.
Our Privacy Officer is responsible for ensuring that all collection, use and disclosure is done under the
authorization of the Health Information Act, this policy, and other applicable law. Our employees are
trained on our authority, and our responsibilities, regarding the collection, use and disclose of your
personal health information.
We support your right to access and request corrections to health information in our custody, subject to
any restrictions set out in the Health Information Act.
